As COVID-19 spread, providers quickly pivoted to deliver virtual care using new technology, workflows and treatment pathways to care for patients remotely. As providers review their use of telehealth now and prepare to move forward with virtual care as a new normal, this session offers insight from a provider organization that has been using telehealth and Facilitated Virtual Care, an innovative new way to bring providers to patients in their homes, for 8 years. During this session attendees will learn how Facilitated Virtual Care can be implemented to: 1. Improve Access to Care: Bring the care/doctor to the patient 2. Reduce inappropriate ED visits and avoidable hospital admissions 3. Improve the health of populations (patient health) 4. Enhance the experience of care for individuals (better care) 5. Reduce the Total Cost of Care 6. Improve Quality and HEDIS scores 7. Increase integration across providers and care settings 8. Advance health equity and address social determinants of health
